The vendor shall provide Nexplanon NDC #0052-4330-01 (or equal) on an open-end and continuing basis. The implant must be radiopaque, a single flexible rod with a maximum measurement of 40mm in length and 2mm in diameter, non-biodegradable, provide protection against pregnancy for three years, have a failure rate of less than 1%, and have a minimum shelf life of three years and a maximum shelf life of five years. The vendor must participate with the 340B Drug Discount Program and cannot set the price higher than the 340B price point. Delivery of standard orders must be within 3 working days and emergency orders within 1 working day, F.O.B. destination.
