sexual violence (length of time providing services, variety of programming offered, etc.).
Include whether you are a Qualified Victims Services Provider as described in OAR
137-085-0060 or have a plan to become one by 7/1/27.
What key strengths, resources and/or abilities does your agency bring in providing
services to survivors of domestic and/or sexual violence?

Please describe how you ensure confidentiality of all personally identifying information
and other information related to program participants and their children/dependents that
meets the VAWA confidentiality requirement. Please include information on your written
subpoena response policy.
Describe how you ensure that staff and volunteers have knowledge, experience and
training on domestic and sexual violence, survivor services, person-centered
communication style (ie. Motivational Interviewing, Assertive Engagement), and trauma
informed care; including assuring they meet the minimum training requirements for
domestic and sexual violence victim/survivor advocacy programs, as established by
OAR 137-085-0060.
What is your policy/procedure for assuring that staff and volunteers complete required
training in a timely manner and are not providing services to survivors prior to obtaining

have participated in other community collaborative efforts to address systemic barriers
and how your would participate as a DSV service provider.
Describe how your agency approaches relationship building and co-advocacy with DSV
continuum partners and other aligned programs.
