Scope of Work: (Contractor must obtain and submit all permits prior to performing any work.)
The scope of work consists of performing Prescribed Maintenance at designated Florida Department of Transportation (FDOT) traffic signal intersections throughout Miami-Dade County. Refer to the attached Prescribed Maintenance Checklist for task details. The Contractor shall provide all materials, tools, labor, and equipment necessary to perform the work detailed in the Prescribed Maintenance
Checklist and as per the requirements of the contract documents.
Traffic signal prescribed maintenance must be performed at a minimum of 60 signalized intersections per calendar month. A list of project locations is included in the contract documents.

Insurance Requirements:
The Contractor shall furnish Certificates of Insurance to the County prior to commencing any operations under this Contract. The certificates shall clearly indicate that the Contractor has obtained insurance, in the type, amount and classification required by these Contract Documents.
-Worker’s Compensation Insurance as required by Florida Statute 440.
-Commercial General Liability Insurance for $1,000,000 each occurrence, $2,000,000 aggregate including Products/completed operations. Miami-Dade County must be included as an additional for ongoing and completed operations.
-Automobile Liability Insurance covering all owned, non-owned and hired vehicles for $1,000,000 combined single limit.
-Umbrella liability for $3,000,000 each occurrence/aggregate. Terms to be not more restrictive than underlying coverages
