Selected Contractors will provide Post-Acute Care Services on a temporary and as-needed basis within the homes of patients or such other locations as instructed by the County facility referring the patients, in accordance with the treatment plan initially established by each patient's County physician. Services are divided into subcategories: Home Health (active) and Hospice Care (suspended). Contractors may seek to qualify for additional subcategories during the term of the Master Agreement.
