Each selected organization will deliver the program and services described in its proposal during Fiscal Year 2028. The organization must expend funds solely for Delaware citizens, accept incorporation of measurable objectives into the award agreement, submit expenditure reports and an annual outcome report, provide an annual certified audit or equivalent financial statements, and participate in the Division's monitoring activities including desk reviews and site visits. Proposals must include a cover letter, statement of need, alignment with Health Fund priorities, program description, measurable outcomes and evaluation plan, organizational capacity, budget and budget narrative, and certifications and attachments.
