The Virginia National Guard is looking for a vendor to provide medical exams to employees who may have been exposed to hazard materials.

civ@army.mil. No phone calls will be accepted. Questions will be received until October 9, 2026,
no later than 11:00 a.m. Eastern Standard Time (EST).
2. ADDENDUM to 52.212-1, Instructions to Offerors -Commercial Items (SEP 2023)
A. Addendum to FAR 52.212-2(b), Submission of offers. The offeror shall submit an electronic
copy of their dated quote via e-mail to: katina.l.moore7.civ@army.mil with the solicitation number
W912LQ-27-A001 in the subject block by October 19, 2026, no later than 11:00 a.m. EST.
Facsimile quotes will not be accepted. Offerors who do not provide all the required information with
their quotes may be deemed NONRESPONSIVE for further consideration for the solicitation and
award. In addition to the items required by 52.212-1(a), the offeror shall complete and provide the
following with their submission:
Signed and completed Information Sheet. (Attachment 1)
Completed Reps and Certs for 52.219-1
Proof of medical liability insurance
Pricing Schedule attached.
The contractor must provide a fully equipped mobile clinic and personnel to perform occupational health medical surveillance exams for full-time and temporary VAARNG employees exposed to workplace hazards. Services include physical exams, blood chemistry panels, urinalysis, blood lead/ZPP testing, pulmonary function testing, vision and hearing (audiogram) screening, and phlebotomy services, performed at locations including Fort Pickett/Blackstone, North Chesterfield, Virginia Beach, Troutville, Sandston, and Gate City, VA. The contractor must submit exam documentation within 30 business days of each event and a master summary report within 3 business days, and must maintain medical liability insurance and ensure staff meet applicable occupational medicine credentialing standards.
