The contractor must provide off-facility comprehensive dental exams, limited exams, consults, radiographs, periodontal services including all preventative procedures, deep scaling root planning, tooth extractions, periodontal procedures, crown procedures, crown lengthening, biopsies, denture procedures, and pathology evaluations for the dental clinic at San Angelo State Supported Living Center. Services are to be performed Monday through Friday between 8 a.m. and 5 p.m. Central Time at the facility located at 11640 US Hwy 87 North, Carlsbad, TX 76903. The contractor must furnish all labor, tools, transportation, equipment, materials, and supplies. The contractor must comply with all applicable laws and maintain a current Texas State Dental Board License, DEA registration, and CPR certification.
