The Connecticut Department of Mental Health and Addiction Services (DMHAS or Department/Agency) is issuing this RFP in collaboration with the Department of Social Services (DSS) to identify nursing home level of care for individuals in state care who are challenging to place. They could be transitioning from a correctional facility, have criminal justice involvement and/or be individuals transitioning from a higher level of care provided by DMHAS. The State is seeking a provider that is clinically qualified, has established connections to the local behavioral health and corrections services landscape and can work effectively across systems.
DMHAS, in consultation with the DSS, will enter into a contract to eventually provide 45 beds for nursing home level of care that will serve individuals who are challenging to place and meet the Preadmission Screening and Resident Review (PASRR) requirements for nursing facility level of care.
