The contractor must supply and deliver three line items: 720 boxes of Heat and Moisture Exchanger (Provox 8262), 2400 each of ONE TOUCH VERIO (GLUCOSE) TEST STRIP, and 400 cases of CATH SET, INTERMIT 14FR H#96144 to the VA CMOP Chelmsford. Delivery is FOB destination. The bidder must be an OEM authorized dealer or distributor and provide OEM authorization documentation. Gray market items are prohibited. Expiration dates of all medical surgical supplies shall be no less than one year from date of delivery.
