LIMITATIONS ON SUBCONTRACTING-CERTIFICATE OF COMPLIANCE
FOR SERVICES AND CONSTRUCTION (JAN 2023) (DEVIATION) pg 30.
The contractor will conduct annual inspections of all fire and smoke rated doors on smoke and fire walls annotated in the Life Safety Plan at the VA San Diego Healthcare System, 3350 La Jolla Village Dr., San Diego, CA 92161. Inspections must be in accordance with NFPA 80, NFPA 105, and The Joint Commission PE.03.01.01 requirements. The contractor will inspect 180 doors within Buildings 1 & 11 Interstitials, 380 doors throughout Buildings 1 & 11 non-Interstitials, and provide up to 100 retests annually. The contractor will complete all inspections by 09/04/2026, report any safety discrepancies by close of business that day, provide a list of findings at the end of each Thursday or by floor, present a separate report of rollup doors, and present final consolidated reports by 09/11/2026. The contractor will also act as an advisor or consultant for VA Life Safety, including three two-hour meetings and five hours of consulting total for survey/verification. The contractor must hold state and federal certificates to work in an asbestos containing area, an asbestos certification of no less than 16 hours and a valid fit test certification, and be certified in NFPA 80 and NFPA 105 Smoke & Fire Doors. The contractor shall provide all necessary tools and Personal Protective Equipment (PPE). Work in Building 1 interstitial spaces requires an asbestos certification of no less than 16 hours and a valid fit test certification. Work in inpatient areas of Buildings 1 and 11 must be performed outside of patient regular sleep hours (1900hrs to 0800hrs). The contractor shall provide initial findings/failures daily and the final written report within 3 weeks after completion of onsite inspection. Reports shall include identification of door on life safety drawing with callout diagram and a spreadsheet with door information; each building's report shall be in a separate 3-ring binder with a one-page executive summary. One digital file shall be emailed or sent via an approved electronic file transfer method.
